For XP Start Menu:

Right-click Taskbar, choose Properties, choose Start Menu tab Click Customize, Advanced tab, and click Clear List button at the bottom.

For Classic Start Menu:

Right-click Taskbar, choose Properties, choose Start Menu tab Click Customize, and click the Clear button

Click OK, OK and close the dialog.
